Drivers Who Have Nighttime Sleep Disturbance (Sleep Apnea) and Use a CPAP Machine:

Bring a reading from your machine indicating you have used it appropriately
If you are being treated for a mental health condition you will need to bring a letter from your mental health professional stating "your condition is stable and all medications are well tolerated and allow you to safely operate a commercial vehicle".
Are you diabetic? Yes
If yes are you taking insulin for your diabetes? Yes
If yes taking insulin, then bring a copy of the MCSA-5870, Insulin-Treated Diabetes Mellitus Assessment Form (click on our links tab to download form if needed)
If not taking insulin but diabetic it is helpful if you can bring a copy of latest blood work results including A1C.
Any surgeries in the last 3-6 months? Yes
If yes what type of surgery?
